feat: support Codex paginated lineage bundles #2

Extend the single-file Codex profile to capture and install paginated forks and reverts with history_base and _<rolloutId> filenames.

The current profile rejects these files because native loading depends on byte-identical base rollouts. Select the complete dependency graph, reject missing or conflicting bases, preserve every file, and define publication/retry behavior before relaxing that rejection. Add pinned-client fork and revert round trips, including collision and interrupted-install cases.

Follow-up to #1. The initial codex/paginated@0.160.0 delivery intentionally remains one self-contained rollout.
